If a parent is not accompanying a minor child to the immunization appointment, please call in advance to receive consent forms or click the link below to access the Consent for Immunization.

Non-Medical Waivers

To obtain a non-medical waiver, you will need to make an in-person appointment with a DIDHD nurse. The non-medical waiver process will be completed in MCIR.
