Michigan Care Improvement Registry (MCIR)

Michigan Care Improvement Registry (MCIR) was created in 1998 to ensure consistent accessibility to a person’s immunization record regardless of the person’s location in Michigan. This system also helps to track local and state immunization rates of Michigan residents to ensure protective coverage of a community to vaccine-preventable illnesses.MCIR benefits healthcare organizations, schools, licensed childcare programs, pharmacies and Michigan’s citizens by consolidating immunization information from multiple providers into one comprehensive immunization record. The MCIR helps healthcare providers view up-to-date patient immunization history in one system.

If you were born before 1998, your immunization record may not have all the vaccines you have received. Historical vaccine entries can be updated in the MCIR system. Individuals interested in having their MCIR updated, must provide proof of vaccination.

  • Visit the MCIR Portal to download your State of Michigan immunization record (must be 18 years or older to access)
  • Visit or call your healthcare provider
  • Visit the Dickinson-Iron District Health Department (must have photo ID to obtain record)
